Goal: TD Green Streets strives to green cities and towns across Canada by supporting innovative practices in municipal forestry.

Impact: TD Green Streets supports innovative practices in community forestry and has awarded funding to more than 500 communities across Canada. In 2012, 25 projects were funded resulting in the planting of 16,344 trees & shrubs nationwide.

Goal: The mission of the Teddy Bear Clinic is to decrease fear of medical professionals and the hospital environment, and to teach healthy habits at a young age.

Impact: The program has been highly regarded throughout the community, and we have had requests from parents outside the community to offer it to their schools as well. We are considering hosting two clinics for other schools to join.

Goal: The State of Arizona's original interest in telecommuting arose out of the search for solutions to common issues facing many areas in the United States: traffic congestion, air pollution, and energy consumption. However, Arizona has found that telecommuting is a powerful management tool that increases employee productivity while reducing the cost of employee turnover.

Goal: Baylor Scott & White started TelePsych because many of its hospitals do not have any psychologists or psychiatrists on staff. The Hillcrest hospital in particular has seen an increase in patients needing acute mental health care. Its location next to the highway and the dearth of other trauma centers means that the hospital receives 1-2 patients per day with acute mental health needs. Hillcrest has licensed medical social workers in the Emergency Department but struggled without an actual psychologist or psychiatrist available. Their goal in implementing TelePsych was to move mental health patients through the continuum of care efficiently and with empathy.

Impact: The hospital staff at Baylor Scott & White Hillcrest has been very impressed with the implementation and ease of use of the TelePsych process and system. Patient satisfaction has increased and staff are able to move patients through the next level of care more efficiently.

Goal: The 16th Avenue Tiled Steps Project was a neighborhood effort to create a mosaic running up the risers of 163 steps.

Goal: The Berkeley Charleston Dorchester Regional Bicycle and Pedestrian Action Plan is based on three principles:

(1) Children should be able to safely walk and bike to school if they and their parents so choose.

(2) Roadways should equally accommodate pedestrians, bicyclists, and motorists.

(3) Bicycling and walking should become a routine part of daily activity in the BCD region.

Goal: The Black Infant Health Program aims to reduce African-American infant mortality through the Social Support and Empowerment model - a faith-based program which seeks to:

- Ensure access to care for at-risk pregnant and parenting women and their infants.

Greater Hampton Roads
