To accomplish our goals, we:
- Collaborate on our Community Health Needs Assessments (CHNAs)
- Identify shared community health needs
- Build strategies to address those needs
Heart Health
2016 CHNAs revealed a shared need to improve cardiac health in Williamsburg. Riverside and Sentara collaborated with a variety of community partners, and the initiative was designated as a HEARTSafe Community in 2018. The alliance creates a "cardiac chain of survival" in Williamsburg. Elements of the partnership include enhanced EMS systems, CPR training, AED distribution, increased public awareness, and secondary health measures to support cardiovascular wellness. As of October 2019, over 900 people have received free training in hands-only CPR.
Obesity
To reduce obesity rates and improve nutrition and wellness, PCHC partners have collaborated to develop food pharmacy programs. Sentara CarePlex and the Virginia Peninsula Foodbank recently piloted a four-week program to provide a comprehensive care plan for heart failure patients. Qualifying patients attend weekly workshops where they participate in nutritional education, view a cooking demonstration, and receive a box of related ingredients in a food box provided by the Virginia Peninsula Foodbank.
Opioid Epidemic
Sentara Careplex Hospital and Hampton Police Division partnered to train 300 Hampton police officers in the use of Naloxone nasal spray. Sentara CarePlex emergency department nursing staff and Hampton EMS conducted training classes for police officers, and the hospital has provided spray devices so the officers can be equipped to administer Naloxone, also known as Narcan, when they are the first response to an individual experiencing an overdose.
Mental Health
PCHC partners collaborate to increase access to mental health services, reduce stigmas surrounding mental health, and improve the overall mental health and wellness of Peninsula residents. Recent initiatives related to mental health include health literacy outreach, partnerships with local justice departments, and patient care coordination.
In 2019, CHKD began construction on a 14-story mental health hospital and outpatient treatment center in downtown Norfolk (view the virtual tour to the left). With an expected opening in 2022, the facility promises to increase residents' access to high-quality pediatric mental health services.
